Late Work Slip
If you have not done so already, please review the late work, make up work, and revision work policies and procedures BEFORE submitting a late work slip.

Please follow the directions on the Late Work Slip carefully. (Preview to the right.) It should be attached to ANY work that is late (submitted for the first time after the posted due date), regardless of reason.
***Do NOT attach a Late Work Slip to work that you want to revise (to correct in order to raise your grade). Revision work has a separate process. Please review the difference under the Policies and Procedures tab. |

How to fill out the form: Choose which column fits you best, working from left to right. It is VERY important that you fill out the appropriate column completely.
***If you have an IEP/504 Plan/active ESOL, you will ALWAYS fill out the first column only.

FIRST column:
Fill out this column if you DO have a legal document stating that you need extended time such as an IEP/504/ESOL, regardless of reason. |
SECOND column:
Fill out this column if you were absent for the assignment or due date AND you do not have a legal document stating that you need extended time such as an IEP/504/ESOL. |
THIRD column:
Fill out this column if you were NOT absent for the assignment or due date AND you do NOT have a legal document stating that you need extended time such as an IEP/504/ESOL. |
Where to submit: ALL late work, with stapled late work slip, should be put into the appropriate class' slot within the secured yellow Late Work Box. Maintaining the confidentiality of the late work reason is extremely important, so once you slip the work into this bin, you may NOT get it back out. Therefore, make sure it is just the way you want it (all parts completed well and labeled with your first and last name, date, block) BEFORE you turn it in. If you are caught trying to go through the work in this box, it is the same as if you were to go through confidential material in my desk and will be written up on a referral to carry the same serious consequences.
What happens next:
Once I grade your work, the slip will be removed and filed with my documentation. I will input your new grade and any comments into Focus. Your work will then be placed in the "To Be Filed" folder in the front of your class' portfolio crate. Finally, as soon as time allows, your work will finally be filed into your portfolio.
